How to Read an NPI Registry Entry

A provider's NPI registry entry — visible on DoctorDataHub — contains a structured set of fields drawn from the NPPES database. Knowing what each field means helps you get the most out of this data.

NPI Number and Entity Type

The 10-digit NPI number is the unique identifier. Entity type tells you whether this is a Type 1 (individual) or Type 2 (organization) provider. This is one of the first things to note — most individual clinicians are Type 1.

Name and Credential

For individual providers, you'll see a first name, last name, and credential field. The credential (MD, DO, NP, PA, etc.) reflects what the provider self-reported when registering. It may not always be fully accurate or up-to-date. Cross-reference with a state license lookup for critical purposes.

Primary Taxonomy and Specialty

The taxonomy code and its label (e.g., "Family Medicine" or "Cardiology") describe the provider's primary specialty. DoctorDataHub uses this to populate all specialty directory pages. A provider can have up to 15 taxonomy codes, but only the primary is featured on browse pages.

Practice Address

The practice address is the location where the provider primarily sees patients. This is distinct from the mailing address (used for correspondence). For hospital-based providers, this will typically show the hospital address. For telehealth-only providers, it may show a state licensing address that doesn't correspond to a physical clinic.
